Position Title: Personal Assistant

Employment Type: Hourly, Non-Exempt

Supervisor: Consumer/Designated Representative

Objective: To deliver support to Consumers in accordance with their Care Plan and directives from the Consumer/Designated Representative.

Key Responsibilities:

  • Follow the Care Plan, which includes tasks such as assisting with dressing, personal grooming, hygiene maintenance, mobility support, toileting, light cleaning, and running errands.
  • Adhere to the schedule set forth by the Consumer/Designated Representative.
  • Ensure effective communication with the Consumer/Designated Representative.
  • Abide by the terms outlined in the Personal Assistant Agreement.

Qualifications:

  • Qualifications tailored to the specific needs of the Consumer as indicated in their Care Plan.

Additional Notes:

  • Position may require lifting as necessary for Consumer assistance. Candidates must meet immunization and health assessment standards, and complete mandatory training upon hiring and annually thereafter. Applicants must be at least 18 years of age.
