Position Overview:
We are seeking a dedicated individual with a passion for supporting Veterans. Candidates with experience in Veteran services are highly encouraged to consider this opportunity.

Key Responsibilities:
The selected candidate will be responsible for:
1. Collaborating with Veterans to create a detailed, person-centered financial plan.
2. Acknowledging receipt of referral documentation promptly.
3. Initiating contact with Veterans within two business days to arrange an initial home visit.
4. Conducting quarterly in-person assessments to ensure the well-being of Veterans and to review their financial plans.
5. Effectively communicating with Veterans and/or their Authorized Representatives, providing guidance when there are challenges in adhering to the financial plan.
6. Continuously assessing, identifying, coordinating, and monitoring necessary supports and services.

Qualifications:
Applicants should possess a valid NYS driver’s license, reliable transportation, and the ability to travel as needed. A flexible schedule is essential to meet the needs of Veterans.

Educational Background:
A degree in Social Work is preferred, although other relevant qualifications in Human Services will be considered. Practical experience may substitute for formal education.

Skills and Competencies:
The ideal candidate will demonstrate proficiency in Microsoft Office applications for document preparation, data management, and spreadsheet creation. A strong sensitivity to the issues faced by Veterans is crucial, along with the ability to engage with a diverse population. Skills in community networking, advocacy, and a willingness to maintain a flexible schedule are highly valued. Exceptional customer service, communication, and time management skills are essential. Familiarity with human services, Independent Living principles, and case management is preferred.

Benefits:
This position offers a 37.5-hour work week with options for a flexible schedule, including:
- Four-day work week
- Alternating four-day work weeks
- Standard five-day work week
