Position Overview:
This role is designed for individuals with a background in supporting Veterans or those who have personal experience as Veterans. The selected candidate will engage with Veterans to create a detailed, individualized spending plan that prioritizes their unique needs.

Key Responsibilities:
The chosen applicant will be responsible for the following:
1. Acknowledge receipt of referral documentation from the Veterans Affairs Medical Center (VAMC) within one business day.
2. Initiate contact with the Veteran within two business days of receiving the referral to arrange an initial home visit.
3. Conduct quarterly in-person assessments to ensure the Veteran's well-being and to review the spending plan.
4. Maintain effective communication with the Veteran and/or their Authorized Representative, providing guidance if there are deviations from the spending plan.
5. Continuously assess, identify, coordinate, and oversee necessary supports and services.

Qualifications:
The ideal candidate should possess:
- A valid New York State driver’s license and reliable transportation.
- Flexibility in scheduling to meet the needs of Veterans.
- A degree in Social Work is preferred; however, relevant education in Human Services will also be considered. Experience may substitute for formal education.

Skills and Competencies:
The candidate must demonstrate:
- Proficiency in Microsoft Office applications for document preparation, spreadsheet management, and data handling.
- Sensitivity to the issues faced by Veterans and the ability to work with a diverse population.
- Strong community networking and collaboration skills, along with advocacy capabilities.
- Excellent customer service, verbal and written communication skills, and effective time management.
- Knowledge of human services, Independent Living philosophy, and case management is advantageous.

Compensation and Benefits:
- Standard work week of 37.5 hours, with flexible scheduling options including a four-day work week or a traditional five-day work week.
- Generous leave policy including 16 vacation days, 12 sick days, and 13 paid holidays, which now includes Eid-Al-Fitr and Yom Kippur.
- 401K plan with up to 10% employer contribution.

Salary:
Minimum Salary: $43,992
Maximum Salary: $43,992

Salary Unit:
