Patient Navigator, Bureau of Public Health Clinics

The City of New York is seeking a highly skilled Patient Navigator to join the Bureau of Public Health Clinics. As a key member of our team, you will play a critical role in promoting healthy communities and providing New Yorkers with the resources needed to make informed health decisions.
Key Responsibilities:
- Oversee caseload of STD clinic patients seeking HIV prevention services and ensure connection to prevention and treatment services.
- Deliver health education and health promotion messages according to STD clinical protocols.
- Coordinate logistics for plan adherence, including appointment reminders, scheduling, medication adherence, and arranging transportation as needed.
- Collaborate with STD clinic staff to identify qualified candidates for programs and perform rapid HIV screening and provide results in a confidential manner.
- Maintain liaison with healthcare providers and other agencies to expedite follow-up and treatment of patients enrolled in programs.
Requirements:
- A baccalaureate degree from an accredited college, including or supplemented by twelve semester credits in health education, or in health, social, or biological sciences.
- Or a baccalaureate degree from an accredited college and six months of full-time satisfactory experience in a health promotion or disease intervention/prevention program.
