Job Title: Patient Navigator, Bureau of Public Health Clinics

Job Summary: We are seeking a highly motivated and experienced Patient Navigator to join our team at the Bureau of Public Health Clinics. The successful candidate will be responsible for overseeing a caseload of STD clinic patients, ensuring they are connected to prevention and treatment services, and promoting health education and health promotion messages.

Key Responsibilities:

  • Oversee caseload of STD clinic patients and ensure they are connected to prevention and treatment services
  • Deliver health education and health promotion messages according to STD clinical protocols
  • Coordinate logistics for plan adherence, including appointment reminders, scheduling, medication adherence, and arranging transportation as needed
  • Collaborate with STD clinic staff to identify qualified candidates for the program and perform rapid HIV screening and provide results and advising in a confidential manner
  • Maintain liaison with healthcare providers and other agencies to expedite the efficient follow-up and treatment of patients enrolled in the program

Requirements:

  • A baccalaureate degree from an accredited college, including or supplemented by twelve semester credits in health education, or in health, social, or biological sciences
  • Or a baccalaureate degree from an accredited college and six months of full-time satisfactory experience in a health promotion or disease intervention/prevention program
  • Or a four-year high school diploma or its educational equivalent and four years of full-time satisfactory experience as described above

Additional Requirements:

  • To be assigned to Assignment Level II, candidates must have at least one year of experience as a Public Health Adviser, Assignment Level I, or at least one additional year of experience as described above
