About the Role

We are seeking a highly skilled Service Coordinator to join our Assertive Community Treatment (ACT) team at Pathways Vermont.

The successful candidate will provide flexible, compassionate services that promote stability, recovery, and community reintegration of individuals experiencing mental health challenges, histories of homelessness, and substance use issues.

As a member of our interdisciplinary team, you will engage in relationship building, provide service from a 'choice' perspective, and utilize client-centered, harm reduction, and strengths-based approaches.

You will also assist clients with referrals to community agencies, provide one-to-one support and counseling, and maintain written and computerized records.

We offer a competitive salary of $20-$21 per hour, with up to an 8.5% increase after one year of employment, as well as a $3000 hiring bonus.
