Pathways Vermont is a dynamic nonprofit agency committed to permanently ending homelessness and offering innovative mental health services.

We are seeking a Forensic Assertive Community Treatment Service Coordinator to join our team in Chittenden County, VT.

This full-time position offers a competitive salary of $20-$22 per hour, plus a $3000 hiring bonus after successful completion of a 60-day orientation period.

Job Description:

  • Engage with clients in a client-centered, harm reduction, and strengths-based approach.
  • Collaborate with clients to develop comprehensive Medicaid-compliant service plans.
  • Assist clients in navigating housing options, life skills development, and landlord/client disputes.
  • Develop knowledge of community resources in Chittenden County and facilitate referrals to relevant agencies.
  • Maintain accurate records and documentation, including case notes, face sheets, statistics, and letters.

Requirements:

  • Bachelor's degree or equivalent work experience preferred.
  • Patient, creative, flexible, compassionate, and sensitive to individuals with disabilities and minority populations.
  • Prior experience working in mental health and/or harm reduction agencies, preferably in community services.
  • Excellent communication, writing, and computer skills.
  • Interpersonal skills, supportive counseling skills, and ability to form relationships with marginalized individuals.
