Pediatric Nurse Case Manager

The Kidz Club PPEC provides daily skilled nursing care for children, ages birth to under 21, who have been diagnosed with medical complexities.
PPECs initiate daily conversation among pediatricians, therapists, and family, providing continuity of care through case management, skilled nursing, and enrichment for the betterment of the child.
As a Registered Nurse Case Manager, you will be responsible for managing the care of pediatric patients, according to their individual needs and care plan using a multidisciplinary method.
You will facilitate referrals to primary providers, specialists, healthcare facilities, and community resources.
The ideal candidate will have a graduate degree in nursing and current RN licensure eligible in the state of Kentucky.
Preferred qualifications include at least six months of current experience as a practicing RN with pediatric experience.

The starting rate is $32.00 per hour, with rate increases based on experience.
