Nurse Case Manager

Job Summary:
The RN Case Manager will facilitate appropriate care coordination to patients and their families in cases where specific problems related to clinical aspects influence effective utilization of inpatient or outpatient treatment. The ideal candidate will possess clinical work experience and have a RN licensure; strong understanding of Swanson's nursing model; have direct patient care experience; have exceptional communication skills (written & verbal); enjoy working independently as well as closely with their co-workers in a supportive team environment; outstanding computer skills (to include Microsoft Office); above average customer service skills; and active listening skills. The starting pay for this position was recently adjusted. The minimum annual salary for this position will now be $62,400/year. All pay rates will be based on candidate's experience. This position is eligible for a recruitment bonus. Additional details will be provided during the hiring process should your application be selected to move forward.
Responsibilities:
• Facilitate appropriate care coordination to patients and their families
• Assist in the formation and/or implementation of the patients discharge plan, as related to clinical assessments
• Serve as a liaison with other departments and agencies to ensure continuity of transition of care
• Make recommendations based on evidence/practices that best aligns with the clinical needs of the patient
• Serve as a resource by providing training and education about case management services
Requirements:
• RN licensure
• Clinical work experience
• Strong understanding of Swanson's nursing model
• Direct patient care experience
• Exceptional communication skills (written & verbal)
• Ability to work independently as well as closely with co-workers in a supportive team environment
• Outstanding computer skills (to include Microsoft Office)
• Above average customer service skills
• Active listening skills
Preferred Education/Experience:
A minimum of three years of experience providing patient care as a BSN, RN.
