Job Title: RN Nurse Case Manager I

Job Summary:

Elevance Health is seeking a highly skilled RN Nurse Case Manager I to join our team. As a key member of our healthcare team, you will be responsible for providing comprehensive care management services to our members with complex and chronic care needs.

Responsibilities:

  • Assess and develop individualized care plans to address member health needs
  • Coordinate internal and external resources to meet identified needs
  • Monitor and evaluate the effectiveness of care plans and modify as necessary
  • Interface with Medical Directors and Physician Advisors on care management treatment plans
  • Assist in problem-solving with providers, claims, or service issues
  • Function as a preceptor for new care management staff
  • Work on special projects as assigned by manager

Requirements:

  • 3 years of clinical experience
  • Current, active, unrestricted RN license from the Commonwealth of Virginia
  • Preferred qualifications include previous case management experience, behavioral health experience, and/or experience working with individuals with intellectual/developmental disabilities
