Job Summary

Columbine Health Systems is seeking a highly motivated and compassionate Social Services Assistant to join our team. As a Social Services Assistant, you will play a vital role in supporting the rehabilitation unit and assisting with admissions and discharges.

Key Responsibilities
  • Support the rehabilitation unit and assist with admissions and discharges.
  • Advocate for residents and their families while at our facility.
  • Collaborate with Social Workers and other facility departments as well as community resources.
  • Assist residents in exercising and protecting their rights.
  • Develop and implement individualized care plans for each resident.
  • Assist residents and their families with resources pertaining to discharge planning and post-discharge planning.
  • Complete timely and accurate case-related documentation and assessments.
  • Coordinate and manage ancillary services.
  • Participate in resident council and neighborhood meetings.
  • Schedule Care Conferences with residents, family members, and staff.
  • Manage social and emotional factors and crisis intervention.
  • Assist with audits.
  • Support the Social Services department with workloads as needed.
  • Perform other duties as assigned, including administrative and social services tasks.
  • Adhere to facility policies and procedures, including HIPAA requirements.
  • Participate in interdisciplinary meetings.
  • Comply with expectations related to being a mandated reporter of abuse and/or neglect.
Requirements
  • Associate's degree in the human services field, with experience in healthcare preferred.
  • Experience working with the geriatric population and individuals with Alzheimer's disease and related memory disorders preferred.
