Job Title: Case Management Coordinator

We are seeking a highly motivated and detail-oriented Case Management Coordinator to join our team at Denver Health. As a Case Management Coordinator, you will play a critical role in providing high-quality care to our patients and ensuring they receive the necessary resources and support to manage their chronic illnesses.

Job Summary:

Under general supervision, the Case Management Coordinator will use a client-centered approach to behavior change and proven intervention models to assist clients recently diagnosed with a chronic illness or clients with prior diagnosis and currently out of care. The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively with clients, healthcare providers, and community resources.

Key Responsibilities:
  • Manage assigned caseload in community, clinic, and correctional care settings for clients recently diagnosed with a chronic illness or clients identified with prior diagnosis and currently out of care.
  • Assure linkage to medical care and evaluate and refer clients for other needs including on-going case management, social support, and psychiatric and substance abuse care.
  • Use tracking systems to document client encounters, outcomes, assessments, follow-ups, and contacts with other agencies.
  • Participate in the preparation and presentation of Linkage to Care data and program results.
Requirements:
  • High School Diploma or GED Required
  • 1-3 years Typical candidate will have at least one year case management experience working with high risk populations for HIV/STDs Required
  • Licenses: Knowledge, Skills and Abilities: Ability to use Microsoft Office. Competency in assessment of client's medical, mental health, and psychosocial needs. Knowledge of available community resources for medical, psychological, and social needs. Knowledge of infectious disease transmission, care, and treatment. Basic competency in motivational interviewing
