About the Role:
We are seeking a highly skilled Care Coordinator to join our team at Community Care of North Carolina Inc. As a key member of our care management team, you will be responsible for coordinating and managing the healthcare needs of our members.
Job Description:
The Care Coordinator will work closely with our members, providers, and community partners to ensure that our members receive the highest quality care and support. This role involves assessing the healthcare needs of our members, developing individualized care plans, and coordinating services to meet those needs.
Key Responsibilities:
- Assess the healthcare needs of our members and develop individualized care plans.
- Coordinate services to meet the needs of our members, including medical, behavioral, and social services.
- Work closely with our members, providers, and community partners to ensure that our members receive the highest quality care and support.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
Requirements:
- A Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) degree from an accredited institution.
- A minimum of 2 years of experience in care management or a related field.
- Excellent communication and interpersonal skills.
- Able to work independently and as part of a team.
