We are seeking a highly skilled and compassionate Ambulatory Care Nurse to join our team at WakeMed Health & Hospitals. In this role, you will have the opportunity to provide exceptional nursing care management services to patients in our community.

Key Responsibilities

 

As an Ambulatory Care Nurse, your primary responsibility will be to provide nursing care management services to assigned populations. You will work collaboratively with physicians to identify at-risk patient populations who require care management services. Your key responsibilities will include:

  • Providing comprehensive nursing assessments and developing individualized plans of care
  • Collaborating with interdisciplinary healthcare teams to coordinate patient care
  • Serving as a patient advocate and communicating effectively with patients, families, and healthcare teams
  • Supporting the mission, values, and management of patient care services

 

Requirements

 

To be considered for this opportunity, you must meet the following requirements:

  • Diploma or Associate's Degree in Nursing required
  • Registered Nurse licensure required
