Position Summary

The Community Support Specialist will assist and empower RISE program graduates as they reintegrate into society after incarceration. Reporting to the Reentry Manager, this role involves working with individuals both before and after their release to eliminate barriers to successful reentry and to facilitate access to community resources and services.

The Community Support Specialist will conduct needs assessments, evaluate reentry and personal action plans, aid individuals in establishing both short- and long-term objectives, provide assistance with job and housing searches, make referrals to comprehensive services, conduct wellness check-ins, facilitate educational sessions, verify employment, and manage various case management responsibilities. This position requires close collaboration with the entire RISE Team to inspire and empower program participants to thrive within their families and communities.

Key Responsibilities

  • Employ both peer support and case management techniques to assist RISE program graduates in their transition from incarceration to community life.
  • Foster and maintain supportive relationships with individuals on your caseload.
  • Develop and sustain positive relationships with Prison Program staff, collaborating closely with the personnel at the facility served.
  • For individuals still incarcerated, maintain regular communication through in-prison engagement and correspondence.
  • Conduct post-release needs assessments to identify service and programmatic requirements for reentering individuals. Connect participants with community resources such as housing, mental health and substance use treatment, public benefits, and healthcare.
  • Review reentry plans to create realistic goals and personal action plans that promote successful reintegration, focusing on five key areas of reentry well-being: healthy thought patterns, meaningful employment trajectories, effective coping strategies, positive social interactions, and constructive relationships.
  • Assist individuals in obtaining and completing necessary documentation for their transition (e.g., transcripts, birth certificates, state IDs, social security cards) and navigate the challenges posed by a criminal background.
  • Act as a liaison among individuals, community organizations (e.g., transitional living facilities, parole officers), and family members to ensure a robust support network, enhancing the likelihood of success and reducing recidivism.
  • Build and leverage partnerships with other organizations involved in reentry initiatives.
  • Maintain a cooperative working relationship with the Nebraska Department of Corrections and its staff.
  • Engage with program participants for wellness check-ins and updates on their reentry and personal action plans.
  • Inform caseloads about RISE Community Events, encouraging their participation.
  • Accurately document and maintain records in the database to track individual progress, including case notes on wellness check-ins, reentry and personal action plans, milestone achievements, attendance, class progress, and employment verification.
  • Participate in staff meetings to share information regarding participants' release/parole dates, needs, and updates on their progress and well-being.

Qualifications

  • High School Diploma or GED required; an associate degree in Social Sciences, Human Services, Criminal Justice, Psychology, or a related field is preferred.
  • Certification as a peer support specialist is preferred.
  • A combination of training, education, or lived experience in programs or organizations serving system-impacted individuals is essential.
  • Familiarity with the jail/prison environment and knowledge of the criminal justice system is preferred.
  • Basic computer proficiency is required.
  • Ability to collaborate effectively with diverse groups.
  • Strong written and verbal communication skills are necessary.
  • Must be adaptable, resourceful, and innovative.
  • Availability to work some evenings and weekends is required.
  • Reliable transportation is necessary (reimbursement provided).
  • Ability to enter the Nebraska Department of Correctional Services as a cleared volunteer is required.
