Job Overview

Position Summary

The Community Support Specialist will provide guidance and assistance to individuals transitioning back into society after incarceration. Reporting to the Reentry Services Manager, this role involves working with clients both before and after their release to eliminate barriers to reintegration and to help them access community resources and services.

The Community Support Specialist will conduct needs assessments, evaluate reentry and personal development plans, assist clients in establishing both short- and long-term objectives, offer support in job and housing searches, make referrals to comprehensive services, conduct wellness check-ins, facilitate educational sessions, verify employment, and manage various case management responsibilities. This position requires close collaboration with the entire Reentry Services Team to motivate and empower participants to thrive within their families and communities.

Key Responsibilities

  • Employ both peer support and case management techniques to assist program participants in their transition from incarceration to community living.
  • Establish and maintain supportive relationships with individuals on your caseload.
  • Collaborate effectively with correctional program staff, ensuring a cohesive approach to participant support.
  • Maintain regular communication with individuals still incarcerated through outreach initiatives.
  • Conduct post-release assessments to identify service needs and refer participants to community resources such as housing, mental health services, and public benefits.
  • Review reentry plans to create achievable goals and personal action strategies, addressing key areas of well-being.
  • Assist clients in obtaining necessary documentation for their transition and navigate challenges related to their criminal history.
  • Act as a liaison among clients, community organizations, and family members to ensure comprehensive support networks.
  • Develop partnerships and collaborate with other organizations focused on reentry initiatives.
  • Maintain a positive working relationship with the Nebraska Department of Corrections and its personnel.
  • Conduct wellness check-ins and provide updates on clients' progress regarding their reentry plans.
  • Inform clients about community events and opportunities for engagement.
  • Accurately document case notes and track the progress of individuals in the database.
  • Participate in team meetings to share updates on clients' needs and progress.

Qualifications

  • High School Diploma or GED required; an associate degree in Social Sciences, Human Services, Criminal Justice, Psychology, or a related field is preferred.
  • Certification as a peer support specialist is a plus.
  • Experience working with individuals impacted by the criminal justice system is desirable.
  • Familiarity with the correctional environment and the criminal justice process is advantageous.
  • Basic computer proficiency is required.
  • Ability to collaborate effectively with diverse populations.
  • Strong communication skills, both written and verbal.
  • Demonstrated adaptability and innovative problem-solving skills.
  • Availability to work some evenings and weekends.
  • Reliable transportation is necessary (reimbursement available).
  • Ability to gain clearance to enter correctional facilities as a volunteer.
