Job Description

We are seeking a dedicated Care Coordinator to serve as the primary point of contact for patients throughout their care journey, as well as for external referral sources. This will involve ensuring seamless communication and coordination between our practice, patients, and referral sources.

As a Care Coordinator, you will be responsible for processing referrals, working with third-party payers to obtain authorization for procedures, and being the liaison between the third-party payers, the patient, and our clinical staff. You will also be responsible for maintaining daily, weekly, and monthly reporting, providing outstanding customer service, and scheduling clinic visits and specialty procedures.

Requirements for this role include a high school diploma or greater, 3+ years of medical office experience or the equivalent, and demonstrated skills in collaboration, teamwork, and problem-solving to achieve goals. You must also have excellent written and verbal communication and listening skills, as well as a high level of integrity and trust-ability to keep information confidential.

Key Responsibilities:

  • Process referrals as generated by our providers.
  • Work with third-party payers to obtain authorization for procedures.
  • Be the liaison between the third-party payers, the patient, and our clinical staff.
  • Maintain daily, weekly, and monthly reporting.
  • Provide outstanding customer service.
  • Schedule clinic visits and specialty procedures.
