Job Overview

The Plaza Assisted Living is seeking a qualified professional for the position of Float Director of Nursing (FDON).

This pivotal management role will involve collaboration across our Plaza communities, focusing on the enhancement and support of the nursing department within the designated community.

In instances where nursing leadership is unavailable, the FDON will act as an extension of the Director of Quality Assurance, ensuring compliance and effective management of the nursing department. Key responsibilities encompass, but are not limited to, assisting the community Director of Nursing (DON) and/or Administrator in adhering to state and federal regulations, as well as community policies, with a primary focus on delivering exceptional care to residents.

The FDON will be expected to travel between assigned communities and will report directly to the community Administrator/DON under the guidance of the Director of Quality Assurance.

Essential Duties Include:

  • Being on-call twenty-four (24) hours a day for assigned communities in the absence of a DON, or as required when a DON is present.
  • Conducting assessments for each resident prior to admission/readmission to evaluate if their care needs can be met by the community, and every six months or as necessary for significant changes in condition.
  • Ensuring the creation and maintenance of Resident Service Plans based on assessments, while developing Care Communication for residents, families, and staff.
  • Managing residents who are out of the facility, including those in hospitals or rehabilitation, to ensure timely completion of readmission assessments and orders.
  • Guaranteeing accurate documentation of care delivery as mandated by physicians, in accordance with federal requirements, state regulations, and Plaza policies; monitoring compliance through the electronic medical records system.
  • Overseeing residents' nutritional needs to ensure they are met according to Resident Service Plans and relevant standards.
  • Conducting regular rounds to observe the efficiency of care delivery, engaging with staff, residents, families, and other stakeholders as appropriate.
  • Supervising the Medication Management Program, which includes assessing residents' ability for self-administration of medication and ensuring proper medication storage and administration.
  • Regularly auditing the medication management program to ensure compliance with federal and state regulations.
  • Managing health promotion and disease prevention initiatives, including immunization administration and infection disease management protocols.
  • Reviewing community Incident Reports and assisting in their resolution and prevention.
  • Assisting in the management of the nursing department, including hiring, onboarding, scheduling, payroll processing, and performance evaluations.
  • Setting up necessary nursing department training, including safety procedures and ongoing skills assessments.
  • Maintaining supplies and managing the staffing labor budget in line with community census.
  • Participating in the development and maintenance of a Quality Assurance Program in collaboration with the Director of Quality Assurance and other staff.
  • Promoting customer service and hospitality, addressing and resolving complaints or concerns from residents or families regarding nursing services.
  • Being present during visits from internal, third-party, and state inspectors.
  • Familiarizing oneself with disaster and fire plans as outlined in the policies and procedures manual, assisting with community emergencies as needed.
  • Adhering to all employment requirements established in the Team Member Handbook and related documents.
  • Performing additional duties as assigned by the community Administrator or direct supervisor.

Supervisory Responsibilities:

  • Participating in the recruitment and selection of nursing and care staff.
  • Scheduling staffing needs within budget constraints while minimizing reliance on third-party agencies.
  • Counseling team members in a fair and consistent manner, adhering to Plaza policies and state/federal employment laws.
  • Supervising nursing and care staff, regularly evaluating their performance and engaging with them frequently.
  • Conducting regular shift reports and meetings to communicate information and address concerns.
  • Ensuring cleanliness and orderliness in resident nursing areas and providing ongoing training for care staff as needed.

Qualifications:

  • Current State of Hawaii Nursing License RN or higher in good standing.
  • Current Tuberculosis Clearance in accordance with State of Hawaii Department of Health regulations.
  • CPR and First Aid Certification.
  • At least one (1) year of experience in a Director of Nursing role within similar communities or equivalent experience.

Preferred Qualifications:

  • A minimum of three (3) years' experience in an assisted living facility or long-term care setting.

Salary is negotiable based on prior experience.
