Job Overview

The Plaza Assisted Living is seeking a dedicated Float Director of Nursing (FDON) to enhance our nursing management team.

This pivotal role will provide support across our Plaza communities, ensuring that the nursing department operates effectively at the designated locations.

In the absence of nursing leadership, the FDON will act as an extension of the Director of Quality Assurance, overseeing compliance and management of nursing services. Key responsibilities encompass, but are not limited to, assisting the community Director of Nursing (DON) and/or Administrator in adhering to state and federal regulations, as well as community policies, with a focus on delivering high-quality care to residents.

The FDON will be required to travel between assigned communities and will report directly to the community Administrator/DON under the guidance of the Director of Quality Assurance.

Essential Duties:

  • Available on-call twenty-four (24) hours a day for assigned community or communities, both in the presence and absence of a DON.
  • Conduct assessments for each resident prior to admission/readmission to evaluate if their care needs can be met by the community, with follow-ups every six months or as necessary.
  • Ensure the creation and maintenance of Resident Service Plans based on assessments, and develop Care Communication for residents, families, and staff.
  • Manage out-of-facility residents, including hospital or rehabilitation stays, ensuring timely completion of readmission assessments and orders.
  • Oversee proper documentation of care delivery as mandated by physicians, in compliance with federal and state regulations, as well as The Plaza policies.
  • Monitor residents' nutritional status to ensure care needs are met, including quality checks for monthly weights and consultations with Registered Dieticians.
  • Perform regular rounds to observe the efficiency of care delivery, engaging with staff, residents, families, and other relevant parties.
  • Supervise the Medication Management Program, assessing residents' abilities for self-administration and ensuring proper medication storage and administration.
  • Conduct routine audits of the medication management program to ensure compliance with federal requirements and Plaza policies.
  • Manage health promotion and disease prevention initiatives, including immunization administration and infection disease management protocols.
  • Review and monitor community Incident Reports, assisting in resolution and prevention efforts.
  • Support the nursing department's operations, including hiring, onboarding, scheduling, payroll processing, training, and performance evaluations.
  • Facilitate necessary nursing department training sessions, including accident prevention procedures and Medication Aide Skills Training Assessments.
  • Maintain supplies and manage staffing budgets in accordance with community census.
  • Contribute to the development and maintenance of a Quality Assurance Program in collaboration with the Director of Quality Assurance and other departmental staff.
  • Promote customer service and hospitality, addressing and resolving any complaints or concerns from residents or families regarding nursing services.
  • Be present during visits from internal, third-party, and state inspectors.
  • Familiarize with disaster and fire plans as outlined in the policies and procedures manual, assisting with community emergencies as needed.
  • Adhere to all employment requirements as established in the Team Member Handbook and other procedural manuals.
  • Perform additional duties as assigned by the community Administrator or direct supervisor.

Supervisory Responsibilities:

  • Participate in the recruitment and selection of nursing and care staff personnel.
  • Schedule staffing needs within budget constraints, minimizing reliance on third-party agencies.
  • Provide fair and consistent counseling to team members, adhering to Plaza policies and state/federal employment laws.
  • Supervise nursing and care staff, regularly evaluating their performance and engaging with team members through various communication methods.
  • Ensure that nursing department areas are clean and orderly, providing ongoing training for care staff as necessary.

Qualifications:

  • Current State of Hawaii Nursing License RN or higher in good standing.
  • Current Tuberculosis Clearance as per State of Hawaii Department of Health regulations.
  • CPR and First Aid Certification.
  • Minimum of one (1) year experience in a Director of Nursing role within The Plaza communities or equivalent.

Preferred Qualifications:

  • At least three (3) years of experience in an assisted living facility or long-term care setting.

Salary is competitive and negotiable based on prior experience.
