Are you a skilled primary care nurse practitioner (NP) or physician assistant (PA) dedicated to enhancing patient outcomes through exceptional care? Join our team at Seattle Post Acute Care Practice as a full-time nurse practitioner (NP) or physician assistant (PA) in a skilled nursing facility, where you will provide comprehensive care to patients five days a week.
Key Responsibilities:- Deliver primary care services in a skilled nursing facility for both short-term and long-term patients.
- Manage chronic illnesses, diagnose medical conditions, and develop effective care plans.
- Collaborate with physicians and other healthcare professionals to ensure coordinated patient care.
- Work effectively within a multidisciplinary healthcare team.
- A minimum of 1 year of experience in primary care is required.
- Experience in post-acute care is advantageous but not mandatory.
