Position Overview

CareSTL Health is seeking a dedicated Referral Coordinator to undertake essential administrative responsibilities, provide guidance to individuals requiring services, and communicate vital information to various organizations.

Key Responsibilities:

  • Manage both internal and external referral authorizations while monitoring specialty care referrals to identify patterns in patient adherence to healthcare provider recommendations, ensuring timely access to necessary care.
  • Support hospitals and insurance providers to secure referral authorizations promptly, aiming for optimal health outcomes for patients and their families.
  • Actively engage in performance improvement initiatives, collaborating effectively with team members and maintaining clear communication with insurers and healthcare facilities.
  • Generate monthly reports detailing the status of all referrals, consistently tracking patient referrals to external agencies and maintaining an organized referral log within the electronic medical record (EMR) system.
  • Coordinate services from multiple agencies to benefit clients, assisting patients in navigating potential challenges related to healthcare, financial, or social barriers.

Additional Responsibilities:

  • Establish connections among service providers and demonstrate the ability to work independently.
  • Encourage client accountability in maintaining appointments.
  • Assess client progress in accessing appropriate medical care and other necessary services.
  • Continuously update technical knowledge and skills relevant to the role.

Qualifications:

  • Education: High School Diploma or GED; completion of an accredited Medical Assistant program is preferred.
  • Experience: 1-2 years in a community health center or medical office environment.
  • Other: Current CPR Certification is required; valid registration as a Registered Medical Assistant in Missouri is preferred. Reliable transportation is necessary.
