Key Responsibilities

The Multi-Systems Specialist, under the guidance of the Multi-Systems Supervisor, plays a crucial role in supporting the planning and staffing initiatives for children and adults engaged with multiple program areas within DHS. The primary focus is to coordinate and execute collaborative planning for individuals involved with various systems, facilitate meetings across diverse settings, and provide technical support in accessing and linking essential resources. Additionally, the incumbent will serve as a representative on various projects across program offices.

  • Willingness to travel periodically, including some overnight stays at group homes, with mileage reimbursement available at .67 cents per mile.
  • Evaluate multi-systems referral data and prioritize referrals to ensure appropriate resource allocation.
  • Organize, lead, and staff multi-systems planning meetings as required.
  • Offer technical assistance and resolve individual cases for youth and adults.
  • Act as a liaison and provide technical support among DHS program offices, service providers, caseworkers, and managers to identify necessary services.
  • Connect with and follow up on resources identified during multi-systems meetings or technical assistance sessions.
  • Ensure that all agreements and action items determined by the multi-systems team are executed.
  • Maintain timely communication with team members regarding completed assignments or objectives.
  • Document and share notes, action items, and data in a centralized database (Synergy, KIDS, MPER) for all individuals served by the multi-systems team on a regular basis.
  • Foster and sustain professional relationships within the multi-systems framework across DHS, providers, and stakeholders.
  • Assist in tracking and planning for youth in probation placements, hospital stays, and residential treatment facility placements.
  • Refer cases to the Multi-Systems Supervisor for Rapid Response referrals, Options Meetings, and Specialized Placement referrals when necessary.
  • Identify cases where multi-systems planning calls cannot yield viable solutions due to resource limitations or regulatory issues, and escalate these to the Multi-Systems Supervisor.
  • Coordinate and facilitate 10-day admission meetings upon entry to specialized group homes and ensure case transfer meetings occur as needed.
  • Adhere to the case transfer process when transitioning cases within the multi-systems team or to other DHS program offices.
  • Collaborate closely with CYF Placement Stability, ODS Promising Practice, and OBH Children’s teams regularly.
  • Participate in CYF and other program office staffing as required.
  • Engage in regular individual and group supervision to address case concerns and progress.
  • Work from the CYF Regional office at least two days per week and travel to other program office locations as necessary.
Supervisory Responsibilities

This position does not entail supervisory responsibilities.

Additional Duties

Occasionally, the employee may be required to undertake additional tasks and responsibilities as directed by the employer.

Qualifications
  • Familiarity with child-serving systems within the Department of Human Services.
  • Understanding of system strengths and weaknesses.
  • Exceptional writing capabilities.
  • Ability to plan, organize, and engage in multi-systems activities.
  • Capacity to manage multiple tasks concurrently.
  • Strong interpersonal and group facilitation skills.
  • Creative problem-solving and mediation abilities.
  • Proficient computer skills for data collection and analysis.
Education and Experience

A Bachelor's degree in social work or a related field, along with a minimum of one year of clinical and case management experience is required. Experience in developing multi-systems service designs is preferred, as well as familiarity with policy, administration, and funding.
