Job Title: Emergency Community Support Specialist

Job Summary:

The Salvation Army USA Central Territory is seeking a highly motivated and compassionate Emergency Community Support Specialist to join our team. As a key member of our Emergency Community Support Program, you will play a vital role in providing intensive case management services to individuals in need.

Key Responsibilities:

  • Participate in discharge planning from higher levels of care and establish a therapeutic relationship with clients.
  • Provide pre-discharge transition services, including teaching daily living skills, scheduling appointments, and transportation to appointments.
  • Complete comprehensive assessments and develop individualized service plans in partnership with clients and their support systems.
  • Collaborate with clients, family members, and other providers to ensure seamless care coordination.
  • Provide education and support to clients and their families, and develop and maintain formal and informal support systems.
  • Respond rapidly to crisis situations and complete safety and aftercare plans within 3 days of intake.
  • Collaborate with clients on wellness recovery action plans and provide advocacy services as needed.
  • Assist clients in accessing benefits, such as Social Security Insurance, housing vouchers, and Medicaid, and provide referrals to community-based behavioral health services.
  • Participate in staff meetings, file reviews, case consultation, performance and quality improvement, and clinical supervision.
  • Provide on-call coverage and complete 15 hours of intensive training per year on topics such as emergency protective custody, board of mental health, and wellness recovery action plan.

Requirements:

  • High school diploma or equivalent required, with at least one of the following: two years lived experience, two years direct care experience in a human service field, two years training in a human service field, or a bachelor's degree in a human service field.
  • Ability to be flexible and work on multiple projects or tasks simultaneously.
  • Knowledge of Microsoft Word, Excel, and email software, as well as the ability to access payroll, timekeeping, and personal data via a web-based system.
  • Administrative skills, including utilization of standard office equipment, data entry, 10-key, attention to detail, and filing.
  • Good communication skills, both written and spoken, and ability to maintain effective working relationships.
  • Demonstrated ability to handle confidential matters and sensitivity to the service population's cultural and socioeconomic characteristics.
