We are seeking a skilled Healthcare Navigator to join our team and help us deliver exceptional care coordination services to high-needs Medicaid members.
Key Responsibilities- Develop and implement individualized care plans to address the holistic needs of our members.
- Communicate effectively with members and families, considering language and cultural preferences.
- Facilitate care planning meetings as part of a multidisciplinary team.
- Bachelor's degree in a related field OR a minimum of 2 years of relevant experience in medical or behavioral health services.
- Excellent written and verbal communication skills.
- Experience in Health Home or similar programs preferred.
- Tax-Exempt Health Insurance Reimbursement Arrangement (HRA)
- Six (6) Federal Paid Holidays
- One (1) Floating Holiday
- Eleven (11) Days Paid Time Off (PTO)
- Seven (7) Days Paid Sick Time Off
