Care Navigator and Advocate

We are seeking a highly motivated and empathetic Care Navigator and Advocate to join our team at ACMH, Inc.

In this role, you will work closely with clients, healthcare providers, and community resources to develop and implement personalized care plans that address their unique needs and goals.

The ideal candidate will have experience in health services navigation, a strong understanding of healthcare systems and policies, and excellent communication and interpersonal skills.

About the Role

As a Care Navigator and Advocate, you will be responsible for:

  • Conducting assessments and screenings to identify client needs and develop personalized care plans.
  • Collaborating with healthcare providers and community resources to ensure timely and effective care delivery.
  • Providing education and support to clients and their families on health-related topics and self-care strategies.
  • Maintaining accurate and detailed records of client interactions and care plan progress.
  • Participating in team meetings and contributing to the development of best practices in care management.
