About the Role:
The Ambulatory Care Navigator at The Brooklyn Hospital Center plays a vital role in coordinating care services to enhance preventive and chronic care outcomes. This position requires strong collaboration with patients, families, and healthcare teams to develop personalized care plans that cater to each individual's needs.
Key Responsibilities:
- Utilize registry information to identify patients with clinically significant chronic conditions and prevalent health issues within our community.
- Cultivate strong relationships with patients, families, and healthcare professionals to provide comprehensive support.
- Develop and implement effective care coordination strategies to optimize patient outcomes.
- Maintain accurate records and reports on patient progress and program metrics.
- Pursue opportunities for professional growth and development in care coordination and ambulatory healthcare settings.
Requirements:
- Bachelor's Degree in a healthcare-related field.
- Minimum of two years of experience in care coordination and/or ambulatory healthcare settings.
- Strong communication, team-building, and advocacy skills.
- Demonstrated proficiency in Microsoft Office applications, including Word, Excel, and PowerPoint.
- Bilingual in Spanish and English is preferred.
Compensation and Benefits:
We offer a competitive salary range of $65,000 - $80,000 per annum, commensurate with experience, as well as a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) matching, and generous paid time off.
