Role Summary
The Mental Health Association in Ulster County, Inc. is seeking a full-time Housing Support Case Manager to join our Residential Program. This role involves managing a caseload of clients who live independently in their own residences and receive rental assistance. The primary focus of this position is to ensure long-term housing stability and maintenance. Responsibilities include assisting clients with rent payments, fostering positive relationships with landlords and neighbors, managing behaviors related to mental health or substance use, creating crisis intervention plans, connecting clients with essential community resources, and supporting their individual housing aspirations.
The typical work schedule is Monday through Friday, from 9 AM to 5 PM.
The starting hourly wage for this position is $18.64.
Key Responsibilities
- Collaborate with the Residential Director and Site Supervisor to achieve program and departmental objectives, ensuring confidentiality for all participants.
- Assist each client in developing and advancing their personalized goal plans.
- Maintain regular contact with housing clients based on their unique needs and circumstances.
- Provide referrals to various services, including healthcare, mental health treatment, substance abuse support, benefits assistance, nutritional guidance, legal aid, and other community resources, while guiding clients through these processes.
- Accurately document all client interactions and maintain thorough records and reports.
Qualifications
- Applicants must possess a High School Diploma and have at least one year of experience in Human Services.
- Strong writing, computer, organizational, and interpersonal skills are essential, along with the ability to prioritize tasks and work independently.
- A clean, valid NYS driver's license with a minimum of three years of driving experience is required.
