Position Overview

The Case Manager is tasked with:

  • Engaging in a multidisciplinary team approach to foster and sustain a therapeutic setting, which includes consultation and collaboration with relevant agency personnel.
  • Overseeing case management responsibilities, including but not limited to admissions, service planning, reunification, discharge planning, and maintaining accurate documentation.
  • Managing a case load of 8-12 children within the program.
  • Exercising flexibility in individual scheduling to meet the needs of clients and children.
  • Effectively communicating and coordinating with Clinicians, Residential Supervisors, Educational staff, Aftercare, Medical personnel, Guardians, and other treatment team members regarding the treatment progress of individual youths.
  • Exhibiting a strong understanding of human services and child welfare principles, which encompasses respecting the dignity, individuality, and privacy rights of clients and their families.
  • Demonstrating proficiency in working with children and families from diverse cultural and ethnic backgrounds.
  • Completing all necessary program documentation accurately and promptly.

Qualifications:

  • A Bachelor's degree in behavioral sciences, human services, or social services fields.
  • Experience in child welfare and/or case management is highly preferred.

Case Managers are accountable for ensuring that services provided on an individualized basis are suitable and thoroughly documented in accordance with relevant regulatory requirements and organizational policies.
