Oakwood Village is seeking qualified candidates for the role of Care Coordinator within our Prairie Ridge senior living community.

The Care Coordinator plays a vital role in delivering both direct and indirect nursing services to residents during their admission and discharge processes.

This position reports directly to the Director of Resident Care Services in Skilled Nursing.


Position Details:
Exempt role, full-time, with standard business hours, Monday through Friday.

Key Responsibilities:
  • Facilitates the admission process, including assessments and resident orientation.
  • Coordinates care plans in compliance with regulatory standards.
  • Collaborates with nursing staff for clinical admissions and assessments within electronic medical records.
  • Conducts daily rounds and engages with nursing staff and social services.
  • Maintains communication with residents to monitor their adjustment and treatment responses.
  • Initiates discharge planning upon admission and collaborates with the interdisciplinary team.
  • Organizes discharge care conferences within 72 hours of admission.
  • Participates in resident care conferences and facilitates necessary education for residents and families.
  • Coordinates telehealth visits and schedules as needed.
  • Collects and analyzes outcome data to ensure quality measures are met.
  • Completes comprehensive reviews of patients readmitted or transferred to the ER.
  • Educates patients and responsible parties on disease management and medication at discharge.
  • Facilitates community reintegration with social services and healthcare partners.
  • Acts as a liaison among patients, physicians, and families regarding care transitions.
  • Engages in ongoing education and participates in staff orientation programs.
  • Performs additional duties as assigned, including emergency response and nursing tasks as directed.

Decision-Making:

Independently makes decisions regarding resident care assignments, documentation, and daily activities.


Interaction:

Engages significantly with various departments, healthcare providers, residents, and their families.


Essential Knowledge and Skills:

Understanding of current nursing practices, applicable laws, and regulations related to resident care.

Ability to provide skilled nursing care, communicate effectively, and meet the physical and cognitive demands of the role.


Qualifications:
  • Graduation from an accredited nursing program and valid RN licensure in Wisconsin.
  • Experience in Long Term Care or as a Nurse Care Coordinator is preferred.
  • Knowledge of Wisconsin regulations and Medicare requirements is advantageous.
  • Ability to work collaboratively in a team-oriented environment.
  • Strong independent judgment and decision-making skills are essential.
