Service Coordinator

At People Inc, we are seeking a highly skilled Service Coordinator to join our team. As a Service Coordinator, you will play a vital role in providing general case management and referral services to our clients.
Key Responsibilities:- Link clients to supportive services and medical services provided by public agencies or private practitioners.
- Assess client needs and determine eligibility for public services.
- Coordinate the delivery of services with state and local agencies.
- Provide formal case management, including assessment, development of individualized service plans, and periodic re-assessment.
- Maintain up-to-date knowledge of senior issues, including state entitlement programs and community services.
- Participate in case conferences and training programs with public and private agencies.
- Provide information and assistance to clients and participate in tenant move-outs as needed.
- Represent clients in benefit eligibility interviews.
- Oversee students, volunteers, and other individuals as assigned.
- Conduct bi-annual surveys to assess client needs and interests.
- Provide information on site from community agencies and members.
- Advocate on behalf of clients and families.
- Educate clients, families, and staff on available community resources.
- Act as a liaison between community agencies, clients, and families.
- Monitor service delivery and complete required documentation.
- Provide crisis management services as needed.
- Bachelor's degree in gerontology, psychology, or counseling preferred.
- Service Coordinator/Case Manager experience with senior population preferred.
- Working knowledge of aging process and community services preferred.
- Ability to organize, plan, and manage caseloads.
- Sound judgment, flexibility, dependability, and good writing skills required.
- NYS Drivers License that meets agency policy required.
- Ability to handle multiple tasks simultaneously.
