Job Summary

We are seeking a highly skilled and dedicated Services Coordinator to join our team at Yamhill County, OR. As a key member of our Intellectual and Developmental Disability Program, you will be responsible for providing case management services to children and adults with developmental disabilities.

Key Responsibilities:
  • Provide case management services to clients, including needs assessments and documentation, development and monitoring of individualized support plans (ISPs), and team facilitation and problem-solving.
  • Provide consumers with information about available services and support.
  • Collaborate with other professionals to ensure comprehensive and coordinated care.
  • Develop and maintain relationships with community resources and service providers.
  • Ensure compliance with relevant laws, regulations, and policies.
Requirements:
  • Bachelor's degree in social work, psychology, counseling, or education.
  • Two years' experience in a human services field, preferably with the Developmentally Disabled population.
  • Basic knowledge of the public system for Developmental Disabilities services in Oregon.
  • Fluency in both English and Spanish.
  • Valid license to drive in Oregon, proof of auto insurance, and an acceptable driving record.
