Evaluates residents' needs, condition, and care, and assists the care plan coordinator in developing nursing care plans for individual residents.
Key Responsibilities:- Carries out administrative and resident care duties as directed by the care team.
- Reviews and updates service plans with the ECC supervisor and other healthcare team members, attending meetings as necessary.
- Completes monthly summaries on each resident, documenting their progress and needs.
- Takes and transcribes physicians' orders to resident medication sheets, treatment plans, and diet lists, in consultation with the Director of Nursing.
- Admits new residents, assessing their needs and reviewing physician orders to ensure proper care.
- Sends diet orders to the dietary department, ensuring timely and accurate delivery.
- Assembles resident charts, attaching required forms and documents.
- Requisitions supplies, equipment, linens, emergency drugs, and other necessary items to meet unit needs, in consultation with the Director of Nursing and Administrator.
- Evaluates and reprimands Certified Nursing Assistants (CNAs) as needed, ensuring high-quality care.
- Performs physical assessments on Assisted Living Facility (ALF) and independent residents, as ordered by physicians or in response to incidents or emergency calls.
- Abides by department policies and procedures, interpreting them for personnel, residents, medical staff, and the public.
- Exchanges nursing reports and counts narcotics at the beginning and end of shift duty hours.
- Answers telephone inquiries, providing accurate and timely information.
- Orders prescribed medications, supplies, and equipment as necessary, in accordance with established ALF policies.
- Requisitions and arranges for diagnostic and therapeutic services as ordered by residents' physicians.
- Completes unit or medical chart forms as required, maintaining accurate and up-to-date records.
- Assists in scheduling appointments and arranging transportation for ALF residents to other departments or facilities and clinics for diagnostic, therapeutic, dental, or medical services, in consultation with the Director of Nursing.
- Discharges or transfers residents, making necessary arrangements and completing required forms, in accordance with facility policy.
- May assist in on-the-job orientation, instruction, and supervision of nursing personnel.
- Maintains a professional attitude and approach at all times on the unit, ensuring high-quality care and personnel interactions.
- Notifies the ALF Administrator when a resident is transferred to another facility, discharged, or expires.
- Ensures established infection control and universal precautions are maintained when performing duties.
- Follows up on new orders, labs, x-rays, or other diagnostic exams on a daily basis.
- Prefills medication boxes for ALF residents and administers medications as ordered by physicians.
- Assists with resident transfers from one level of care to another within the OLT complex.
- Documents all activities, writes incident reports, and provides a detailed report at the end of each shift for the next nurse to be aware of situations that may occur and have occurred.
- Performs other duties as assigned by the care team.
