Position Overview:
The RN Care Coordinator plays a crucial role in evaluating and addressing the needs of patients and their families. This position involves utilizing the nursing process to coordinate care plans in collaboration with the Interdisciplinary Group (IDG), ensuring that patients receive the palliative and supportive care they require.
Key Qualifications:
- Active RN license in the relevant state.
- At least one year of nursing experience in an inpatient or hospice setting.
- Familiarity with EMR/EHR systems.
- Valid driver's license and automobile insurance as per company policy.
- Reliable transportation for patient visits.
- Proficient communication skills in English, both verbal and written.
- Flexible availability to meet the needs of patients.
- Ability to manage the emotional challenges of working with terminally ill patients.
- Physical capability to navigate various environments and perform essential job functions.
- Current BLS certification from a recognized provider.
Responsibilities:
- Conduct thorough assessments to identify patient and family needs.
- Develop and implement individualized care plans in collaboration with the IDG.
- Educate patients and families about care processes and available services.
- Document all patient interactions and care provided in the EMR.
- Facilitate communication between patients, families, and healthcare professionals.
- Participate in the evaluation of hospice referrals and admissions.
- Provide bereavement support to families as needed.
- Ensure compliance with all regulatory and company policies.
Physical Requirements:
While performing the duties of this role, the following physical abilities are necessary: the ability to see, hear, speak, walk, and use hands for various tasks. Frequent activities include standing, sitting, reaching, lifting, and bending. Occasional activities may involve pulling, pushing, kneeling, and climbing stairs.
