Position Overview:
As a vital member of the healthcare team, the Patient Care Coordinator plays a crucial role in enhancing patient experiences and ensuring the delivery of exceptional care.

Key Responsibilities:
In collaboration with the Nurse Director, you will be responsible for planning, organizing, and executing patient care strategies within your designated unit. Your focus will be on delivering consistent, high-quality care while continuously improving patient safety and satisfaction metrics.

Accountability:
You will oversee healthcare outcomes for a specific patient group, utilizing evidence-based practices to design, implement, and assess individualized care plans. This role includes supervising a team of Registered Nurses, Licensed Practical Nurses, and Patient Care Assistants, ensuring adequate staffing and resources to maintain superior patient care standards.

Qualifications:

  • Experience: Minimum of 1-2 years of relevant experience; preferred 2-4 years.
  • Licensure: Must possess a current Florida Registered Nurse License (RN) or an approved multi-state license.
  • Certifications: Basic Life Support (BLS) certification from the American Heart Association is required, along with Advanced Cardiac Life Support (ACLS) certification.
  • Education: An Associate's Degree or equivalent from a technical school or college is required.
