Position OverviewThis role serves as the primary contact for newly diagnosed individuals with HIV who are enrolling in Cooper's medical care services. Responsibilities include re-engaging individuals who have lost connection to care and providing partner services to both new and existing HIV-positive patients. Collaboration with the medical care team is essential to offer patient counseling for the management of their medical treatment plans.
Utilizing social networking strategies, the position aims to connect new and existing HIV-infected patients with additional social contacts for HIV testing. Prevention counseling and strategies such as CLEAR, ARTAS, or Partners in Health will be provided for patients and their families.
This role is crucial in the execution of a multi-disciplinary care approach.
Required ExperienceA minimum of one year of experience in delivering HIV prevention or care services to individuals living with HIV is necessary. Experience in counseling HIV-positive clients on accessing available services is essential. Familiarity with community resources is also required.
Educational QualificationsA Bachelor of Science in Nursing (BSN) is required, along with a current New Jersey Registered Nurse (RN) License.
Licensure/Certification RequirementsCurrent New Jersey RN License and BSN are mandatory.
Special RequirementsTraining for clinical navigators from the Division of HIV, STD, and TB Services (DHSTS) will be provided.
