Salary:
$70,000 - $80,000 annually


Position Summary:
The primary role involves delivering expert nursing care to clients across various age groups within their homes. This includes collaborating with the interdisciplinary team, patients, families, and referring agencies to ensure comprehensive care coordination.

Reporting Structure:
Reports directly to the Director of Nursing or Director of Patient Services.


Qualifications:


Education:
Must be a graduate from an accredited Diploma, Associate, or Baccalaureate Nursing program.


Licensure:
Must hold a current state license as a Registered Nurse and possess a valid State Driver's License.


Experience:
A minimum of two years of experience as a Registered Nurse in a clinical setting is preferred, with a focus on hospice or home health care.


Skills:
Proficient nursing skills in accordance with accepted standards of practice. Exceptional interpersonal abilities and effective communication skills are essential. Proof of Hepatitis and COVID consent/declination is required. Specialized skills in palliative and end-of-life care are preferred.


Transportation Requirements:
Must have reliable transportation and valid auto liability insurance.


Work Environment:
This position involves working in patients' homes under various conditions, with potential exposure to blood, body fluids, and infectious diseases. Flexibility in scheduling and local travel is necessary, with possible exposure to inclement weather.


Physical and Mental Demands:
The role requires prolonged periods of sitting and walking, with the ability to lift up to 50 lbs. and assist patients. Candidates must be able to work under stress to meet deadlines and patient needs, making quick decisions and resource allocations as necessary.


Key Responsibilities:
1. Conduct initial and ongoing comprehensive assessments of the patient's physical, functional, psychosocial, and environmental needs.
2. Implement and adjust the individualized Plan of Care (POC) as required.
3. Educate patients and families about the disease process, dietary needs, self-care techniques, and end-of-life care.
4. Provide training and support to other staff members as needed.
5. Initiate appropriate preventive and rehabilitative nursing procedures.
6. Document clinical and progress notes that reflect progress towards established goals.
7. Coordinate patient and family services, prioritizing needs with the interdisciplinary team.
8. Utilize a case management approach and refer to additional services as necessary.
9. Communicate changes in patient needs and evaluate responses to care.


On-Call Responsibilities:
Perform on-call duties and provide necessary services to patients and families as assigned.
