Work Schedule: Monday to Friday, 8 AM to 5 PM, with a rotating on-call requirement.

Key Responsibilities:
- Manage a caseload of patients and families, overseeing all phases of the nursing process including assessment, eligibility determination, admission, planning, implementation, and evaluation.
- Gather and evaluate essential information regarding the patient's physical, psychological, social, and spiritual needs, facilitating interventions by the interdisciplinary team.
- Communicate effectively with physicians, team members, and external agencies to coordinate comprehensive care.
- Conduct regular visits to reassess patient needs and adjust care plans accordingly.
- Perform clinical pronouncements of death and provide compassionate support to families.
- Maintain accurate and timely patient documentation, including records, charts, and care plans.
- Supervise and document the care provided by Home Health Aides in compliance with regulatory standards.
Required Qualifications:
- Graduation from an accredited nursing program.
- Current licensure as a registered nurse in the relevant state.
- Valid CPR and First Aid certifications.
- Familiarity with quality assessment and assurance protocols.
- Possession of a valid driver's license and proof of insurance.
