As a Nurse Care Coordinator, you will work closely with both internal and external healthcare teams to provide comprehensive care for clients with or at risk for complex medical issues. Your responsibilities will include creating tailored treatment plans, leading health education sessions, and overseeing hospital admissions and discharges for clients enrolled in our care program. Key Responsibilities:

  • Design and execute personalized treatment plans for clients with co-occurring chronic conditions.
  • Lead health education sessions, utilizing in-depth knowledge of complex medical issues.
  • Oversee hospital admissions and discharges for clients enrolled in our care program.
  • Provide education and training to clients regarding complex health conditions, medical diagnoses, treatments, and medications.
  • Implement and utilize health technology programs effectively.
  • Monitor and report on performance metrics and outcomes using advanced nursing information systems.
  • Collaborate with multidisciplinary teams to ensure comprehensive patient care.
  • Develop and execute wellness and prevention initiatives.
  • Participate in mandatory meetings and quality assurance activities.
  • Adhere to operational guidelines and professional ethical standards.

Qualifications:

  • Active RN license required.
  • Experience with the IDD (Intellectual and Developmental Disabilities) population is preferred.

Additional Skills:

  • Knowledge of chronic disease processes in both children and adults.
  • Strong written and verbal communication abilities.
  • Capacity to implement innovative systems and make informed decisions based on critical thinking.
  • Successful completion of background checks, including criminal and driving records.
  • Valid driver's license and acceptable driving record.
  • Completion of New Hire Orientation and all required training upon employment.
