• Develop and execute personalized treatment plans for clients with co-occurring chronic diseases.
• Facilitate health education sessions, utilizing advanced knowledge of complex medical conditions.

• Provide education and training to patients regarding complex health conditions, medical diagnoses, treatments, and medications.

• Monitor and report performance metrics and outcomes using advanced nursing information systems.
• Collaborate with multidisciplinary treatment teams to ensure comprehensive patient care.

• Adhere to operational guidelines and professional ethical standards.
Education and Experience Requirements:
• Active RN license.
• Experience with the IDD (Intellectual and Developmental Disabilities) population is preferred.
Additional Qualifications:
• Knowledge of chronic disease processes in both children and adults.
• Excellent written and verbal communication skills.
• Ability to implement innovative systems and make decisions based on critical thinking and nursing processes.
• Successful completion of background checks, including criminal record, driving record, and abuse/neglect screening.
• Current driver's license, acceptable driving record, and current auto insurance.
• Completion of New Hire Orientation and all training requirements, including Relias, at the beginning of employment and annually thereafter.
