About the Role

The Clinical Care Coordinator is a nursing specialist who plays a vital role in ensuring seamless patient care across the continuum. This position requires a high level of clinical expertise, as the coordinator will work closely with patients, their families, physicians, and other healthcare professionals to coordinate treatment and care.

Key Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of patients
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination
  • Provide education and support to patients and their families to promote health and wellness
  • Facilitate communication between patients, families, and healthcare providers to ensure timely and effective care
  • Monitor and evaluate patient outcomes to identify areas for improvement
Requirements
  • 2 years of nursing experience required
  • Formal patient education and community educational presentations experience preferred
  • Active RN license in state of practice required
  • Graduate from a school of nursing required
  • Advanced Practice/master's degree in nursing preferred
  • BLS certification required within 30 days of hire
