Key Responsibilities:
- Design and implement personalized treatment plans for clients with co-occurring chronic conditions.
- Lead health education sessions, leveraging advanced knowledge of complex medical issues.
- Oversee hospital admissions and discharges for clients in the Healthcare Home program.
- Educate and train patients on managing complex health conditions, treatments, and medications.
- Utilize health technology programs to enhance care delivery.

- Develop and execute wellness and prevention initiatives.

Qualifications:
- Active RN license.
- Experience with the IDD (Intellectual and Developmental Disabilities) population is preferred.
Additional Skills:
- In-depth knowledge of chronic disease processes in both children and adults.

- Capacity to implement innovative systems and make informed decisions based on critical thinking.
- Successful completion of background checks, including criminal and driving records.
- Current driver's license and auto insurance.
