Key Responsibilities:
- Develop and execute personalized treatment plans for clients with co-occurring chronic conditions.

- Lead health education sessions, leveraging advanced knowledge of complex medical issues.

- Manage hospital admissions and discharges for clients in the Healthcare Home program.

- Provide education and training to patients regarding complex health conditions, treatments, and medications.

- Implement and utilize health technology programs such as Cyberaccess, Care Manager, and IntellectAbility.

- Monitor and report on performance metrics and outcomes using advanced nursing information systems.

- Collaborate with multidisciplinary teams to ensure comprehensive patient care.

- Develop and implement wellness and prevention initiatives.

- Participate in mandatory meetings and quality assurance processes.

- Adhere to operational guidelines and professional ethical standards.

Education and Experience Requirements:
- Active RN license.

- Experience with the IDD (Intellectual and Developmental Disabilities) population is preferred.

Additional Qualifications:
- Understanding of chronic disease processes in both children and adults.

- Strong written and verbal communication skills.

- Ability to implement innovative systems and make informed decisions based on critical thinking and nursing processes.

- Successful completion of background checks, including criminal records, driving records, and abuse/neglect screenings.

- Valid driver's license, acceptable driving record, and current auto insurance.

- Completion of New Hire Orientation and all required training, including annual updates.
