Job Summary

Fairbanks Native Association seeks a skilled Case Manager II to provide case management services for project participants. This role is part of a collaborative treatment team and serves as a liaison between consumer and clinical staff, ensuring culturally resonant intervention, treatment, and support services.

Key Responsibilities

  • Develops individualized Case Management Plans that include access to care, cultural competency guidance, trauma-informed care, workforce development & training, and consumer-paced sustainability.
  • Assesses System of Care (SOC) collaboration with the Behavioral Health Community Coalition (BHCC) and coordinates activities and evaluates outcome annually.
  • Makes appointments for allied and support care.
  • Follows-up on referral care.
  • Works with consumers to understand and address risk factors and connects consumers with resources and support services.
  • Assists with Medicaid and health insurance eligibility, linkage to primary and dental care, and linkage to community organizations through the BHCC.
  • Participates in continuous quality improvement and data-driven decision-making processes to assess program efficacy and drive quality improvement.
  • Ensures adherence to data collection requirements and enters accurate and timely data into the Electronic Health Record (EHR).

Requirements

  • Bachelor's degree in behavioral health or related field.
  • A combination of experience, education, and training that provides the required knowledge, skills, and abilities may substitute for degree requirements.
  • Experience working with electronic health records.
  • Master's degree in behavioral health or related field.

Additional Information

This position requires a criminal background investigation and pre-employment drug testing. Continued employment status will be contingent upon satisfactory completion of these requirements.
