Case Manager II Position

Key Responsibilities:
- Develop individualized case management plans for consumers, including support services and care coordination.
- Work with consumers and the treatment team to promote overall health and well-being.
- Collaborate with the My Place Rapid Rehousing project to locate stable housing for consumers.
- Assist consumers with enrollment in allied services and coordinate health insurance eligibility.
- Maintain accurate and timely data in the Electronic Health Record (EHR).
Requirements:
- Bachelor's degree in behavioral health or a related field.
- One year of experience in case management, preferably working with the Alaska Native/American Indian population.
- Ability to work independently and collaboratively as part of a team.
Preferred Qualifications:
- Master's degree in behavioral health or a related field.
- Two years of experience in case management.
- Experience working with the LGBTQIA community.
