Job Title: Exceptional Needs Care Coordinator

Marion County is seeking a highly skilled and compassionate Exceptional Needs Care Coordinator to join our team. As a key member of our Adult Behavioral Health team, you will provide critical services to individuals with serious and persistent mental illnesses and complex needs.

Job Summary:

The Exceptional Needs Care Coordinator will work closely with community and state partners to assess individuals' needs, determine eligibility for services, and authorize enrollment for Choice Model services. This role requires strong clinical skills, excellent written and verbal communication, and the ability to work collaboratively with a multidisciplinary treatment team.

Key Responsibilities:
  • Act as a liaison between ABH and various stakeholders involved in mental health care, including Oregon State Hospital, acute care hospitals, and coordinated care organizations.
  • Collaborate with individuals in the mental health system, partners, and stakeholders to assess individuals' needs, determine eligibility for services, and authorize enrollment for Choice Model services.
  • Attend OSH Interdisciplinary Treatment Team (IDT) meetings for individuals and participate in Ready to Transition determination with OSH.
  • Facilitate community transition for individuals who fall under Choice criteria and provide case-specific consultation to providers and community partners.
  • Collaborate with acute care staff regarding care recommendations, diversion opportunities, and Long-Term Psychiatric Care needs.
  • Ensure accuracy of utilization data of those admitted to the Oregon State Hospital and acute care hospitals and provide routine reports to Marion County Management.
Requirements:
  • Master's degree in psychology, social work, recreational therapy, music therapy, art therapy, or a behavioral science field.
  • One year of postgraduate degree experience in the behavioral health field.
  • Must be licensed by the Oregon Board of Licensed Professional Counselors, licensed by the Oregon Board of Licensed Social Workers, or credentialed or registered through the Mental Health and Addiction Counseling Board of Oregon (MHACBO) as a Qualified Mental Health Professional (QMHP) within the first 30 days of employment.
