Job Title: Exceptional Needs Care Coordinator

Marion County is seeking a highly skilled and compassionate Exceptional Needs Care Coordinator to join our team. As a key member of our Adult Behavioral Health team, you will provide exceptional care and support to individuals with serious and persistent mental illnesses and complex needs.

Job Summary:

The Exceptional Needs Care Coordinator will provide assessment and treatment services, crisis intervention, and treatment planning for individuals and their families. This role requires strong clinical skills, excellent communication, and collaboration with community partners and stakeholders.

Responsibilities:
  • Provide assessments and treatment planning for individuals with complex needs
  • Collaborate with community partners and stakeholders to ensure seamless care coordination
  • Develop and implement individualized treatment plans
  • Provide crisis intervention and support services
  • Work closely with the multidisciplinary treatment team to ensure comprehensive care
Requirements:
  • Master's degree in psychology, social work, or a related field
  • One year of postgraduate degree experience in the behavioral health field
  • Strong clinical skills and excellent communication
  • Ability to work collaboratively with community partners and stakeholders
