ICAN Supported Employment Program
Position Summary:
The Case Manager plays a crucial role in the administration of client services within the Supported Employment Program. This position is dedicated to assisting clients in achieving their service objectives and personal goals, aligning with the mission of ICAN. The ideal candidate will be organized, adaptable, detail-oriented, and reliable.
Key Responsibilities:
- Manage daily operations related to client services, ensuring effective support for individuals with developmental and physical disabilities.
- Conduct intake assessments for new clients and facilitate meetings with existing clients and support staff.
- Process and maintain accurate documentation and reports for assigned clients.
- Foster an environment that promotes client choice and empowerment.
- Report directly to the Supported Employment Program Manager.
Work Schedule:
This is a full-time, in-person role with a standard schedule of Monday through Friday, 8:30 AM to 5:00 PM.

Qualifications:
- Minimum age of 21 years.
- Legal eligibility to work in the United States.
- Consistent attendance and punctuality are essential.
- Strong communication skills to interact with clients, guardians, and various stakeholders.
- Ability to multitask and work independently while meeting deadlines.
- Comfortable working with individuals with disabilities to help them achieve their employment aspirations.
Education and Experience:
- At least two years of college education in a human services field or equivalent experience in a service delivery system for individuals with disabilities.
- A minimum of one year of experience working with individuals with developmental disabilities.
- Proficient in computer usage, including Apple Macs, Microsoft Office, Google Docs, and client record management systems.
- Strong customer service and problem-solving abilities.
- Fluency in English with excellent writing and grammar skills; bilingual abilities in Spanish, Chinese, Japanese, Korean, or ASL are advantageous.
Licensing and Certification:
- Ability to pass a tuberculosis skin test.
- Completion of fingerprint clearance prior to employment.
- CPR and First Aid certification required or willingness to obtain.
- CPI certification or willingness to complete prevention training upon hire.
Professional Development:
- Participation in state-mandated ongoing professional development and continuing education is required, with a minimum of eight hours annually.
Essential Duties:
- Review and reconcile client case notes daily to ensure accuracy and address any concerns.
- Oversee the intake process for new clients, ensuring timely documentation.
- Facilitate meetings for goal reviews and client progress assessments.
- Develop personalized plans for clients based on their skills, education, and career aspirations.
- Maintain comprehensive client records and regularly update the database.
- Prepare reports aimed at enhancing client independence and employment opportunities.
- Monitor completion of required documentation for clients and funding sources.
- Conduct annual client satisfaction surveys in compliance with program standards.
Team Collaboration:
- Act as a liaison for client care, addressing goals and behavioral issues.
- Maintain positive relationships with clients, families, and service providers.
- Report any client issues to the program manager with proposed solutions.
- Participate in team meetings and training sessions to enhance service quality.
- Ensure compliance with all relevant standards and regulations.
- Identify and address barriers to client success in achieving their objectives.
- Serve as a positive role model within the ICAN community.
