Job Title: Certified Peer Support Specialist

At Southeastern Integrated Care LLC, we are seeking a Certified Peer Support Specialist to join our team. As a Peer Support Specialist, you will play a vital role in providing coaching, mentoring, and consultation to individuals in recovery, promoting self-advocacy, self-direction, and wellness.

Key Responsibilities:
  • Provide individualized coaching and mentoring to clients, focusing on recovery, self-advocacy, and self-direction.
  • Collaborate with interdisciplinary healthcare teams to promote a culture of recovery and empowerment.
  • Develop and implement personalized recovery plans, incorporating client goals and preferences.
  • Provide linkage to community resources and support services, ensuring seamless transitions and continuity of care.
  • Document direct service delivery, interventions, and client progress, adhering to Medicaid and CSEUC standards.
  • Participate in staff and supervisory meetings, training, and development activities, ensuring ongoing professional growth and development.
Requirements:
  • High School Diploma/GED
  • NC Certified Peer Support Specialist certification
  • Valid driver's license
  • Ability to read medical records, write complete service notes, and effectively present information to clients and stakeholders
Work Environment:

The work environment is primarily in clients' living environments and in the community, with occasional exposure to domestic animals, pests, and other environmental factors.

Physical Demands:

The physical demands of this role include standing, walking, reaching, and lifting up to 20 pounds, with occasional lifting up to 50 pounds.
