As a Registered Nurse in this position, you will oversee and guide patients and their families through the various stages of care, utilizing state-of-the-art in-home technology and protocols in conjunction with a multidisciplinary healthcare team. Please note that this role is not a remote position.

Key Responsibilities:

  • Deliver direct nursing care in line with established guidelines and protocols.
  • Continuously monitor and document patient conditions, ensuring effective communication with the healthcare team.
  • Evaluate and adjust care plans to meet the evolving needs of patients.
  • Act as the primary liaison among various healthcare disciplines to ensure seamless patient care.
  • Administer prescribed medications and treatments according to nursing standards.
  • Assess and coordinate discharge planning needs with the healthcare team.
  • Provide education and resources to patients and their families, ensuring understanding of care plans.
  • Engage in ongoing professional development through formal and informal education.

Qualifications:

  • Graduation from an accredited nursing program; BSN preferred.
  • Experience in critical care settings is advantageous.
  • Current state licensure as a Registered Nurse (RN).
  • Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) certification preferred.

Work Environment:

  • Requires the ability to work in a fast-paced and sometimes stressful environment.
  • Physical demands include lifting, standing for extended periods, and exposure to various health-related conditions.
