Schedule: Monday, Tuesday, Thursday 8:00am-5:00pm. Wednesday & Friday 10:00am-7:00pm

Position Summary: The Advanced Practice Clinician is responsible for delivering primary care medical services to patients in alignment with their training, experience, and licensure. This role involves collaboration as a key member of the medical team within the organization. The individual will also serve as an educational resource for patients, the organization, students, and the community.

Key Responsibilities:

• Engage in direct patient care, which may encompass obstetrics, family planning, pediatric care, and general adult health services.

• Achieve minimum patient care productivity benchmarks as established by the organization to meet external funding and reimbursement criteria.

• Deliver continuous, comprehensive, and culturally competent healthcare of the highest standard.

• Manage cases for patients within the organization.

• Ensure thorough documentation in patient medical records, including education, medications, referrals, diagnostic tests, and follow-up care.

• Assist in organizing and participating in specialized clinics, such as those for migrant populations.

• Provide daily oversight of support staff regarding individual patient management.

• Engage in case management discussions and reviews, including for obstetric patients and those with chronic conditions.

• Support the implementation of plans to achieve the objectives of the annual health initiatives and other goals set by the quality assurance committee and the organization’s managed care strategies.

• Participate in chart audits to evaluate the effectiveness of the quality assurance program.

• Attend medical staff, obstetrics, quality assurance meetings, and other committees aimed at enhancing patient care and the overall functioning of the organization.

• Supervise students and residents in healthcare disciplines. Engage with community groups or agencies to facilitate the organization’s integration into the community.

• Assist the Medical or Executive Director in fulfilling the organization’s objectives.

• Perform additional duties as assigned.

• Handle protected health information (PHI) in compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

HIPAA Compliance:
The licensed independent healthcare provider will have access to PHI while performing their duties. The provider will utilize PHI for treatment and quality assurance activities. In adherence to the minimum necessary standard of HIPAA, the designated records accessible to this employee will include demographic and scheduling information in the practice management system, as well as the complete medical record.

Required Knowledge, Skills, and Abilities:

• Bilingual proficiency in Spanish and English is preferred; willingness to attain sufficient Spanish language skills to provide medical care.

• Strong dedication to working with patients and colleagues from diverse ethnic, economic, cultural, and social backgrounds.

Education and Experience Requirements:

• Completion of a family nurse practitioner or Physician Associate training program accredited by the State of Oregon.

• Licensure by the State of Oregon through the Board of Nursing or the Board of Medical Examiners and DEA registration.
